Provisioning hangs if one validation exists for a set of more than one domain

To reproduce:

  1. Make sure on my-demo.site resolves properly and that www.my-demo.site does not.
  2. Start process to provision certificates for my-demo.site and www.my-demo.site.

What happens:

We get the validation for my-demo.site but we end up waiting forever for the validation for www.my-demo.site.

On subsequent attempts, we get the validation back for my-demo.site but it hangs.

What should happen:

On initial run

We should time out while waiting for the second validation.

On subsequent runs

We should try to validate the unvalidated domain again.
